Aspen Family Medicine and Geriatrics began in 2002 as John A. Fagan Family Practice, later rebranding to Aspen Family Medicine & Geriatrics. For over twenty years, Aspen Family Medicine & Geriatrics has proudly served the community of Rancho Cucamonga and surrounding areas, providing quality and compassionate care to our patients. In May of 2024, Aspen Family Medicine & Geriatric’s welcomed Dr. Renier Rosquete Mendez to join the team. Reviews from patient’s hail Dr. Mendez to be attentive and thorough in tending to his patients, making them feel looked after. With his proficient interpersonal skills and ability to speak both English and Spanish fluently, it has allowed him to connect with a broad community in the Inland Empire. Aspen Providers, comprised of Dr. John Fagan, Dr. Renier Rosquete Mendez, and physician assistants: Jocelyn Tomista, Meredith Dettmar, Dana Nemou, Chelsea Henry, and staff are dedicated and driven, constantly aspiring to make every patient like family. We hope to help you find answers to your medical questions and to provide you and your family with the quality medical care you deserve!

Personal Goal: Build enduring partnership with my patients. Also help patients prevent, understand, and manage illness, navigate the health system, and set health goals.

Jocelyn Tomista, PA

Medical School: Western University of Sciences, Maters of Sciences, Physicians Assistant.

Training: Family Practice, Pediatrics Dermatology.

Meredith Dettmar, PA
